Socrates has more than 80 commonly used Scores and patient surveys available for use. They include the commonly used pain, function, quality of life and satisfaction scores, as well as patient reported medical and current injury questionnaires. Almost all are web based to allow the patients to enter their scores on line as well as having scannable forms available if the web can’t be used.
There are a number of child/adolescent scores included such as the IKDC adolescent and the KOOS child scores, these are found in the modules relating to their condition. There are also several Quality of Life scores and surveys which are included in all modules.
We have also developed some military specific surveys for the US Society of Military Orthopaedic Surgeons (SOMOS).
Note that the use of the scores and surveys in Socrates does not imply that users have a license or permission to use them. Sites need to ensure that any surveys/scores they use have the appropriate approvals and licenses for their intended use, and that they comply with the conditions of use if the survey is free to use. Any applicable fees are paid to the licensor.
