Scannable Forms

For patients who don’t have an email address, or can’t manage to use the internet SOCRATES®  has developed a proprietary method of enabling the completed paper forms to be entered by scanner. This is included in the cost of SOCRATES® , there is no expensive 3rd party software company involved. The patient checks the boxes, or draws a line on the scale, the program reads the response into SOCRATES® and calculates the score. There are warnings if there are missing or ambiguous responses

Advantages are:

• Uses a standard document scanner - these are inexpensive and it doesn’t have to be used exclusively for SOCRATES®
• Reduces manual input and is more accurate.

• Saves time, a 4 page form can scan and populate the screen in 30 seconds, add the date and you’re done
• All the scannable forms are included in the program, they can be printed from the screen of each score so you will never run out of forms.
